These terms govern the use of the appointment intake and confirmation automation ("Service") operated by Fabio Gabriel LLC ("we," "us") on behalf of our clinic and business clients. By submitting an intake form or replying to a message sent through the Service, you agree to these terms.
Nature of the Service
The Service performs administrative scheduling functions only: capturing appointment requests, sorting them by urgency for front-desk triage, registering them with our client's systems, and sending confirmations. The Service does not provide medical advice, diagnosis, or treatment of any kind. Any classification or priority note generated is administrative only and must be reviewed by our client's staff.
SMS Program Terms
Program name: FG Automation Appointment Confirmations
Description: Text messages confirming that an appointment request has been received, and related administrative updates.
Message frequency: Varies; typically one message per appointment request.
Message and data rates may apply.
Opt-out: Reply STOP at any time to stop receiving messages.
Help: Reply HELP, or contact info@fgabrielfabrication.com, for assistance.
Carriers are not liable for delayed or undelivered messages.
No Medical Advice
Nothing sent or received through the Service constitutes medical advice or a diagnosis. If you are experiencing a medical emergency, call 911 or your local emergency number immediately.
Accuracy of Information
You are responsible for the accuracy of the information you submit through an intake form. Appointment requests are not confirmed until you receive an explicit confirmation from our client's staff.
Limitation of Liability
The Service is provided on an "as is" basis for administrative convenience. To the fullest extent permitted by law, Fabio Gabriel LLC is not liable for missed, delayed, or misrouted appointment requests, or for decisions made based on automated classification.
Changes to These Terms
We may update these terms from time to time. The "last updated" date above reflects the most recent revision.
Governing Law
These terms are governed by the laws of the State of Florida, United States.
