When a patient or lead submits an intake form on behalf of one of our clients, we collect the information provided, which may include:

- Reason for visit / appointment request details
- Insurance provider (if applicable)
- Preferred method of confirmation (SMS or email)

Information is used strictly to operate the scheduling automation:
- To register the appointment request in our client's system
- To perform administrative triage/priority sorting (never a clinical diagnosis)
- To notify our client's front desk of a new request
- To send the patient a confirmation of their appointment request, by SMS and/or email
SMS Communications
By submitting your phone number and selecting SMS as your confirmation preference, you consent to receive text messages related to your appointment request. Message frequency varies based on your activity (typically one confirmation message per request). Message and data rates may apply. Reply STOP at any time to opt out of further messages. Mobile Information & Text Messaging — Non-Sharing Statement
We do not share, sell, or rent mobile phone numbers or SMS opt-in consent with third parties or affiliates for marketing or promotional purposes, under any circumstances. Mobile information collected in connection with text messaging consent is used only to send you the appointment-related messages described in this policy.
Other Information We Collect
Details submitted through an intake form (such as name, reason for visit, or insurance provider) stay between Fabio Gabriel LLC and the specific client business you are booking an appointment with (e.g., your clinic), solely to fulfill that appointment. We never sell this information, and we never use it for third-party marketing.
Data Retention & Security
Information is stored in secured, access-controlled systems (including encrypted databases) used solely to operate the automation described above. We retain information only as long as needed to support the scheduling relationship with our client, or as required by law.
Your Choices
You may opt out of SMS messages at any time by replying STOP. You may request access to, correction of, or deletion of your information by contacting us using the details below.
